The U.K. Maine Accident Investigation Branch (MAIB) released its Report on the Investigation
of the grounding of the M/V LYSFOSS in the Sound of Mull, Scotland on May 7, 2001. The primary causes of the grounding were determined to be that the chief officer was alone on the bridge and the voyage was not properly planned.
