Grounding Blamed on Poor Bridge Teamwork

The UK Marine Accident Investigation Branch (MAIB) issued its report on the investigation of the grounding of the chemical tanker ATTILIO IEVOLI in the west Solent on 3 June 2004. The grounding caused bottom plate indentation. There were no injuries and no pollution. The report indicates that the primary cause of the grounding was poor bridge team management.
